Will I experience scarring?
Older less refined hair transplant procedures required invasive surgeries involving scalpels and sutures. For example the “strip mining” procedure would involve cutting a literal strip of scalp out of the donor area of the patient’s head. The wound would be closed with stitches or medical staples, and the excised strip would be mined for living follicles to be transplanted to the recipient areas.
Another procedure involved cutting plugs – small circular patches of scalp – out of the donor area. These were then grafted onto the recipient area. Other destructive procedures involved cutting areas of the scalp featuring still-growing hair and twisting or otherwise manipulating them into the areas of the scalp suffering from hair loss. In any of these cases the hair transplant before after pictures reveal unsightly scars.
What about coverage?
In some cases these scars can be unmistakable, especially after the recipient area had endured more natural hair loss, leaving less original hair to camouflage the injured zones of the scalp. Aside from scarring, the other ongoing complaint patients of these earlier procedures often had is that the coverage from the transplanted hair was spotty.
The most obvious example of spotty coverage relates to the plug method of hair transplantation. Once the remaining original hair coverage in the donor area died off the plugs would be left exposed and extremely obvious. There was no way for patients to style their hair to disguise the plugs. They were forced to make a choice of shaving it all off, wearing a hair piece or hat, or to simply sport the poor and obvious results from their transplant surgery.

The above procedures were harmful to patients. There was both the physical risk of the actual invasive operations, as well as the social risks involved with carrying the unmistakable look of a disfiguring and unsuccessful hair transplant surgery.
